503B INSPECTION RESPONSE RECORD · FULL CAPA AUTHORIZATION ARC

The investigator walks the full CAPA arc. This is the record that survives every step.

Initiation. Action plan. Extensions. Effectiveness. Each is an authorization moment. Each is something an investigator can challenge. Most QMS systems document the action. Few document the reasoning behind the call.

This record is generated from your structured inputs only. ComplianceWorxs does not invent evidence, fabricate effectiveness conclusions, or substitute generic compliance language for reasoning. Gaps are flagged. Authorization belongs to the named decision-maker — not the platform. Your inputs are encrypted in transit and at rest, stored under access controls, and used only to generate your authorization record. ComplianceWorxs does not have access to your facility data or quality systems.

Step 1 of 6 CAPA Identity

RECORD — IN PREPARATION

Four authorization moments. One inspection-grade record. ~12–18 minutes.

DECISION TYPE — LOCKED CAPA Full Authorization Arc · Initiation · Action Plan · Extensions · Effectiveness

Step 1 of 6 · CAPA Identity

Identify the record under inspection.

Basic identifiers an investigator will use to locate this CAPA in your QMS. ~90 seconds.

Step 2 of 6 · Initiation Authorization

Why was the CAPA classified, scoped, and root-caused this way?

The first authorization moment. Investigators challenge severity, root cause acceptance, and scope independently of closure. ~3 minutes.

Step 3 of 6 · Action Plan Authorization

Why this plan, timeline, and owner?

The second authorization moment. The plan an investigator will compare to the alternatives you considered. ~3 minutes.

Step 4 of 6 · Extension Authorization

Did the CAPA close on the original target?

Extensions are the highest-exposure authorization moment in the arc. If none were issued, you state that affirmatively below. If any were issued, each is its own authorization record.

Step 5 of 6 · Effectiveness Authorization

Why was closure justified at this point?

The reasoning the investigator interrogates hardest. Convergent evidence, explicit exclusions, recurrence hypotheses ruled out, residual exposure. ~5 minutes.

Convergent evidence streams (minimum 3 independent streams)

Absence-of-deviations alone is not sufficient. The strength of the authorization is in convergence.

Evidence explicitly not relied upon

Evidence reviewed but consciously rejected. Preserving exclusion logic makes reasoning reconstructable in both directions.

Alternative recurrence hypotheses ruled out

The investigator will reconstruct failure modes. Show you reconstructed them first.

Step 6 of 6 · Authorization & Delivery

Final authorization chain and regulatory anchors.

The signature trail across all four moments and the regulations your reasoning aligns to. ~2 minutes.

Regulatory anchors (select all that apply)

You'll receive a structured PDF authorization record ready for QMS upload (Veeva, MasterControl, TrackWise, ETQ). Filename: IRR-{CAPA-ID}-{YYYYMMDD}.pdf. CAPA ID, decision owner, and regulatory anchors embedded as PDF metadata.

You'll see a preview of the record and gap analysis across all four authorization moments before payment. Payment unlocks the full record.

The next inspection is the one you cannot reconstruct for.
